Screening Questionnaire for Perceived Quality Experiment
The purpose of this screening questionnaire is to see if you meet the criteria for taking part in our research study. We are running this study to help us learn more about the impact of perceptual artifacts on perceived quality. The methods used in this study are all intrinsically safe and pose minimal to no risk to our study participants. You may choose not to complete this questionnaire or not to take part in the screening procedure at any time. If you qualify and are chosen for the study, you will be scheduled for a sessions lasting 60-90 minutes, and you may be asked to take part in up to 10 sessions. [Optional insert: To participate in the session(s), you will be asked to travel to Redmond, WA during normal business hours.] Information about you that you give during this questionnaire will be kept as confidential as possible as required by law. The screening questionnaire is not designed to ask for sensitive personal information, but it is possible that some people may feel uncomfortable answering these questions. Your name and contact information will not be collected unless you qualify to take part in the study and are interested in taking part in it. This information will be kept confidential. This questionnaire is not part of the study; it is only to see whether or not you qualify to take part in the study. Time spent completing this questionnaire is not considered time spent participating in the study.
